Подагра: современные принципы диагностики и лечения

Резюме.
Подагра является системным заболеванием, которое возникает в результате отложения кристаллов моноурата натрия в тканях. К факторам риска относится гиперурикемия, генетическая предрасположенность, возраст, мужской пол, избыточная масса тела, диетические особенности, употребление алкоголя и другие факторы.
Клиническая картина подагры включает бессимптомную гиперурикемию, острый подагрический артрит, межприступный период и хроническую тофусную подагру. «Золотым стандартом» диагностики является идентификация кристаллов монурата натрия в синовиальной жидкости или аспирате тофуса с помощью поляризационной микроскопии. Кроме того, используются методы визуализации, такие как рентгенография, ультразвуковое исследование, компьютерная томография, магнитно-резонансная томография и позитронно-эмиссионная томография.
Основные принципы ведения пациентов с подагрой включают в себя купирование острых приступов подагры, лечение хронической подагры и сопутствующих заболеваний, а также профилактика обострений. Лечебные мероприятия включают соблюдение диеты, изменение образа жизни и применение лекарственных средств для снижения уровня мочевой кислоты в сыворотке крови до целевых значений. Это приводит к растворению кристаллов моноурата натрия и предотвращает обострения заболевания.
