Эозинофильная патология толстого кишечника

Резюме.
В статье представлен обзор литературы по проблемам этиологии, патогенеза, диагностики и менеджмента эозинофильной патологии толстого кишечника и описание клинического случая с эозинофильной инфильтрацией слизистой оболочки толстой кишки из собственного опыта авторов статьи. Число сообщений об «эозинофильном колите» (ЭК), диагностированном по наличию эозинофильной инфильтрации слизистой толстой кишки у пациентов с кишечными симптомами, значительно увеличилось за последние 10-15 лет. Консенсус в отношении его диагностики и менеджмента отсутствует. Диагностика первичного ЭК основана на биопсии толстой кишки, что является особенно сложной задачей в отсутствии строгих диагностических критериев и требует внимательного исключения вторичных причин. ЭК чаще следует рассматривать как неспецифическую воспалительную реакцию, учитывая большое количество известных факторов, связанных со вторичной эозинофильной инфильтрацией толстой кишки, в том числе паразиты и лекарственные средства, неспецифичность клинических проявлений и отсутствие корреляции между симптомами и плотностью эозинофильного инфильтрата. Терапевтические подходы к ЭК основаны на анализе небольшой серии клинических случаев. Необходимы рандомизированные контролируемые клинические исследования, чтобы определить четкие клинические и патоморфологические диагностические критерии ЭК и стратегию его менеджмента.
