Dynamic electroneurostimulation in the complex treatment of infectious and inflammatory diseases of the maxillofacial area and the neck

Abstract.
Objectives. To analyze the effectiveness of the inclusion of dynamic electroneurostimulation (DENS) in the complex treatment of patients with acute odontogenic osteomyelitis complicated by cellulitis of adjacent tissue spaces.
Material and methods. A comprehensive survey of 176 patients with infectious and inflammatory processes of the maxillofacial area was conducted. The observed patients were divided into groups: the control group – 158 persons, the main group – 18 subjects. The reference group consisted of 50 healthy individuals. Clinical evaluation of the provided treatment was carried out on the basis of the state of purulent wounds. The duration of the patient’s treatment and the duration of the given antibiotic therapy were also evaluated. The analysis of blood microcirculation indicators was made. A comprehensive assessment of the immune status was also carried out in the course of treatment based on the results of the blood immunogram, the content of IL-8, IF-γ, TNF-α, PCT in the blood serum and oral fluid. To assist the patients of the main group, DENS was additionally performed on corporal and acuricular acupuncture points in accordance with the localization of the process.
Results. The inclusion of DENS in the medical complex allowed to achieve an earlier relief of pain during palpation of the inflammatory focus (6 (5-7) days), recovery of the face configuration on the 6th (6th-8th) day. Skin hyperemia and purulent exudation from the wound stopped earlier: by 1.5 and 1 day, respectively, and the growth of granulations was determined more quickly – on the 7th (6th-7th) days. At the same time, normalization of erythrocyte deformability in the blood plasma and immunogram parameters by the end of treatment was revealed. The local immunomodulatory effect of DENS was reflected in the cytokine profile of the oral fluid, making it possible to normalize γ-IF, IL-8 and TNF-α, which proves the positive effect of DENS on the immune system of patients with infectious and inflammatory diseases of the maxillofacial area and the neck. The effectiveness of the inclusion of DENS in the complex treatment of patients with acute odontogenic osteomyelitis of the jaw, complicated by cellulitis of the adjacent tissue spaces, was confirmed by the reduction in the duration of treatment by 2 days.
