Factors influencing the serum total testosterone levels in males with acute infectious pulmonary destructions

Abstract.
Objectives. To study the serum total testosterone levels in males with acute infectious pulmonary destructions (AIPD) and to determine their relationship with age, duration, forms, local and systemic complications of the disease, comorbidity pathology, trophological status.
Material and methods. 64 males with AIPD and 25 healthy male subjects were examined. The study of the serum total testosterone levels was carried out with the use of the radioimmune analysis method.
Results. Testosterone deficiency was found in 77% of patients with AIPD. The lowest serum total testosterone levels, compared with lung abscesses (U=112, pMann-Whitney=0.088) and gangrenous abscesses (U=221.5; pMann-Whitney=0.045), as well as the highest frequency of androgen deficiency (89%) (pFisher=0.018) were observed in gangrene of the lung. Serum total testosterone level decreased (H=13.65; pKruskal-Wallis=0.018), the incidence of its deficiency increased (H=13.11; pKruskal-Wallis=0.022), the number of blood lymphocytes decreased (H=18.08; pKruskal-Wallis=0.0028) and the neutrophil-to-lymphocyte ratio grew (H=11.91; pKruskal-Wallis=0.036) with the development and increase in the severity of AIPD complications. There was a direct moderate correlation of serum total testosterone levels with the number of blood lymphocytes (rS=0.46; p=0.00013) and an inverse moderate correlation with the neutrophil-to-lymphocyte ratio (rS=-0.41; p=0.00077) in patients with AIPD. The correlation between the body mass index and serum total testosterone levels was lost in patients with AIPD.
Conclusions. The most significant factors affecting serum total testosterone levels and the frequency of androgen deficiency are the form and AIPD complications (F=2.55; p=0.089 and F=2.34; p=0.056 respectively). The serum total testosterone level in patients with AIPD may be considered as one of the indicators of the severity of the disease.
