Endometriosis: modern principles of diagnosis and treatment (literature review). Part 1. Therapy

Abstract.
This review analyzes and summarizes the current recommendations of the World, European and a number of National Professional Associations of Obstetricians-Gynecologists, as well as Cochrane systematic reviews on the treatment of patients with endometriosis. The «Relevance and Prevalence» section provides the authors’ data on the incidence of endometriosis among patients in a modern gynecological hospital. The main approaches to the diagnosis of the disease are briefly presented. The «Treatment» section analyzes and describes in detail the principles and methods of conservative and surgical treatment for endometriosis. Modern approaches to hormonal therapy are considered, with particular attention focused on the use of first line drugs. The issues of surgical treatment of endometriosis symptoms are also described in detail: indications, approaches, surgical tactics, pre- and postoperative medicamentous therapy for endometriosis surgery. The principles and methods of surgical treatment for endometriosis-associated infertility are examined as well, including the rules of laparoscopic surgery for infertility and the medical tactics after endometrioidectomy. Attention is paid to the possibilities of assisted reproductive technologies in endometriosis. The final part of the review presents the principles of long-term management of female patients, methods of prevention of endometriosis recurrence.
