The influence of treatment tactics of the patients with mandibular fractures on the indices of oral cavity resistance

Abstract.
Objectives. To evaluate the effect of treatment tactics on the indices of oral cavity resistance in mandibular fractures.
Material and methods. To achieve the set goal 231 patients with diagnosed uncomplicated mandibular fractures (MF) without concomitant diseases were examined. The patients were divided into 2 groups: group 1, that included 122 patients with MF, and group 2, that included 109 patients. Group 1 patients were treated with cephalosporin antibiotics, additional therapy included the administration of bifidumbacterin. The treatment of patients in group 2 included: azithromycin, serratiopeptidase, pyobacteriophage polyvalent sextaphage, probiotic florbiolact. 15 healthy volunteers constituted the control group. In dynamics on days 0, 1, 7, 14 and 30 of treatment, the level of lysozyme, the level of immunoglobulin sIgA, and the neutrophil phagocytic activity index of the oral fluid were studied.
Results. The level of lysozyme, the index of phagocytic activity of neutrophils, the level of sIgA immunoglobulin throughout the entire observation period, starting from the moment when the patient  sought medical care and up to 30 days of treatment, remains different from those of healthy individuals. At the same time, in group 2, no statistically significant differences (p>0.05) were found when comparing the studied indicators of the oral fluid with regard to those of healthy individuals.
Conclusions. The scheme of treatment for mandibular fractures of patients, that includes the use of azithromycin, serratiopeptidase, polyvalent sextaphage pyobacteriophage, florbiolact probiotic, contributes to the normalization of the level of lysozyme, sIgA, and the index of phagocytic activity of the oral fluid neutrophils to the level of healthy individuals, which testifies to a positive effect on the resistance of the oral cavity. Consequently, the possibility of developing inflammatory complications of mandibular fractures reduces.
