The correlation of perioperative factors and the volume of blood loss in patients during cardiac surgical interventions

Abstract.
Objectives. To determine the effect of perioperative factors on blood loss in patients requiring open heart surgery with the use of cardiopulmonary bypass (CPB).
Material and methods. A prospective non-randomized cohort study was conducted for determining the correlation of perioperative factors and the volume of intra- and postoperative blood loss in 114 patients undergoing cardiac surgery.
Results. A correlation was found between the volume of perioperative blood loss and the duration of the operation (R=0.28, p=0.002), the time of CPB (R=0.30, p=0.001), and the time of myocardial ischemia (R=0.26, p=0.005). Moderate correlation of intraoperative blood loss was found with CPB time (R=0.33, p=0.02). There is no relationship between perioperative blood loss and the dose of heparin (R=0.031, p=0.74) and protamine sulfate (R=0.14, p=0.16), as well as with intraoperative blood loss – the dose of heparin (R=0.073, p=0.64), and protamine sulfate (R=0.086, p=0.58). Weak correlation was revealed between preoperative platelet level and the volume of perioperative blood loss (R=0.22, p=0.02). We didn’t detect any correlation of preoperative fibrinogen concentration with the perioperative one (R=0.11, p=0.27), as well as with intraoperative bloodlosses (R=0.16, p=0.31).
Conclusions. We have identified three key factors that significantly affect the volume of blood loss during operations with CPB – the duration of the operation, the time of cardiopulmonary bypass, the time of myocardial ischemia. The doses of heparin and protamine sulfate administered during the operation do not affect blood loss.
