Infective fungal endocarditis in a kidney transplant recipient: clinical case

Abstract.
The most common fungi which cause endocarditis are species of the genus Candida, usually isolated from surgically removed emboli or resected valves. In this article a case of prosthetic valve endocarditis in a renal transplant patient on immunosuppressive medications is presented. Endocarditis caused by Candida albicans, was managed with a second-generation azole – voriconazole, which was prescribed based on the drug sensitivity and resistance determined. Delay in antifungal therapy is fraught with disease progression and a fatal outcome. Prosthetic mitral valve damage had a relatively positive dynamics, despite an embolic episode. The difficulty in managing the patient was his comorbidity, the development of an adverse event in the form of eosinophilia against the background of the initial empirical antibacterial therapy.
