The association between parent-child relationships and the internal disease picture in patients diagnosed with ulcerative colitis

Abstract.
Objectives. To identify the association between parent-child relationships and the internal picture of the disease in patients with ulcerative colitis.
Material and methods. Analysis, generalization, interpretation, TOBOL clinical test method for diagnosing the type of attitude to the disease, created by the NIPNI, the author’s questionnaire for determining the relationships in the patient’s parental family, methods of mathematical statistics (descriptive statistics, Kolmogorov-Smirnov criterion, Spearman rank correlation coefficient, ANOVA analysis of variance). The sample consisted of 30 patients of the gastroenterology department of the City Clinical Hospital No. 8 in Izhevsk diagnosed with nonspecific ulcerative colitis (UC) (47% males, 53% females, the average age was 46.7 years).
Results. The importance of childhood experience in shaping attitudes toward the disease was established: physical violence and conflict resolution strategies in the parental family are significant predictors of specific types of attitudes toward UC in adulthood. In addition, socio-demographic factors (education level, age (indirectly, through the duration of the disease)) also demonstrate stable relationships with the types of internal picture of the disease. At the same time, the characteristics of the patient’s current family (composition, cohesion, conflicts) did not show a significant impact on the formation of attitudes toward the disease in this study.
Conclusions. This analysis confirms the importance of psychosocial factors, especially childhood experience, in shaping the patient’s attitude toward a chronic somatic disease, which is important for the development of individualized approaches in psychotherapy and medical support. The results of the study may be of interest to practicing physicians, clinical psychologists, students and teachers of medical universities.
