Compositional hemogram indices in patients with COPD

Abstract.
Objectives. To determine changes in composite hemogram indices, including serum C-reactive protein (CRP) and albumin concentrations in patients with COPD and to determine their relationship with the severity of pulmonary obstruction and the frequency of previous exacerbations during, before and after a treatment of the current exacerbation.
Material and Methods: Sixty-one patients diagnosed with COPD and 27 healthy controls were examined before the start of treatment and after 14 and 90 days correspondently. Cell count and ESR were determined in the venous blood samples, CRP and albumin levels were measured in the serum. The results were analyzed using nonparametric descriptive statistics.
Results. Severe obstruction (FEV₁<50%) gives the greatest increase in the NLR (neutrophils (Nf)/lymphocytes (Lf)), SII (Nf×platelets /Lf and SIRI (Nf x monocytes (Mon))/Lf indices, on average, 2-2.5 times higher than normal. Frequent exacerbations (≥2) even with relatively preserved respiratory function are accompanied by an increase (4.7 times higher than normal) in the CAR (C-reactive protein/Albumin) index. Moreover, it responds best to therapy, judging by its decrease. None of the markers of systemic inflammation (especially CAR and NLR) are completely normalized even 90 days after the start of treatment. During the treatment period (after 14 days), Pt/Mon demonstrates a decrease by 35%-40% mainly due to an increase of monocytes concentration.
Conclusions. The results of dynamic changes in NLR, SII, and SIRI support the hypothesis of chronic systemic inflammation in COPD that is resistant to standard therapy, the severity of which depends on both the degree of obstruction and the exacerbation phenotype.
