Efficiency of conservative therapy and combined approaches in the treatment of chronic subdural hematomas: comparative study results

Abstract.
Chronic subdural hematoma (CSDH) is a common neurosurgical condition in the elderly. This article presents the clinical presentation, diagnostic results, and treatment of CSDH.
Objectives. To substantiate the effectiveness of conservative therapy (alone and combined with surgical treatment) in patients with CSDH, determine its advantages in reducing complications, relapses and the surgical and intensive care unit workload, and study the clinical features and prospects for patient management.
Material and methods. The study (November 2021 – December 2025) included 168 patients with CSDH. In group 1 (n=53) surgery without pathogenetic therapy was carried out. In group 2 (n=64) the endoscopically guided surgery was performed combined with specialized conservative therapy. In group 3 (n=51) only the specialized conservative therapy was applied. All patients were assessed using accepted clinical and instrumental examination methods.
Results. Neurological manifestations varied: pyramidal hemisyndrome prevailed in the surgical groups, while ataxia was present in the therapeutic group. The change in the volume of CSDH after treatment decreased: in group 2 – by 6.1 times, in group 1 – by 1.5, in group 3 – by 5.3 (differences between groups 2 and 3 are significant). According to the Glasgow Outcome Scale, groups 2 and 3 showed an advantage over group 1. Good recovery: group 3 – 100%, group 2 – 93.8% (p=0.262), group 1 – 56.6% (p=0.001). Mortality: group 1 – 17%, group 2 – 3.1%, group 3 – 0% (p=0.002 for 1 vs 3, p=0.203 for 2 vs 3). Postoperative mortality was 15% and 2.9%, respectively.
Conclusions. The performed comparison of the clinical material results demonstrated a new possibility of successful treatment of CSDH using the method of conservative therapy and its combination with surgical methods. Therapeutic treatment of CSDH in clinically compensated patients is an alternative to surgery, allowing for the avoidance of postoperative complications and relapses and reducing the surgical and intensive care unit workload of a medical institution.
