Improving skills of using nonsteroid anti-inflammatory drugs in clinical practice

Abstract.
Nonsteroidal anti-inflammatory drugs (NSAIDs) remain among the most widely used medications in clinical practice due to their anti-inflammatory, analgesic, and antipyretic effects. However, their use is associated with a significant risk of adverse drug reactions, including potentially life-threatening complications, which limits long-term therapy. In this review particular attention is paid to risk factors for NSAID-associated complications and to patients’ stratification according to gastrointestinal and cardiovascular risk. The review outlines contemporary strategies for the prevention of NSAID-induced gastrointestinal and cardiovascular complications during long-term therapy, including rational drug selection and the use of gastroprotective agents. Individualized NSAID therapy based on a comprehensive risk assessment may improve treatment safety and clinical outcomes.
Keywords: nonsteroidal anti-inflammatory drugs, NSAIDs, cyclooxygenase, COX-1, COX-2, adverse drug reactions, gastrointestinal complications, cardiovascular risk, gastroprotection, risk stratification, long-term NSAID therapy.
