Abstract.
Introduction. Acute paraproctitis (anorectal abscess) is a common pathology in emergency proctology. Understanding the anatomy of the anorectal region is crucial for planning an appropriate drainage strategy and reducing the risk of complex rectal fistula formation. Closure of extensive wound defects in the perineum after abscess drainage remains a significant challenge. Objectives. To evaluate the feasibility and initial efficacy of using the autologous stromal-vascular fraction of adipose tissue in the treatment of extensive perineal wounds after surgical treatment of severe acute anaerobic paraproctitis. Material and methods. From October to November 2025, local administration of the autologous stromal-vascular fraction of adipose tissue was being used to treat two patients with extensive granulating perineal wounds following incision of acute pelviorectal paraproctitis in Vitebsk Regional Specialized Clinical Center. Liposuction was performed on the inner thigh under spinal anesthesia. Autologous lipoaspirate was injected para- and intravulnarly. Results. The first experience in the combined use of the autologous stromal-vascular fraction of adipose tissue in treating patients with extensive granulating perineal wounds following incision of acute pelviorectal paraproctitis demonstrated the efficacy of the latter. No adverse effects related to the procedure of autologous fat collection and administration were recorded. Pain at the lipoaspiration site was noted, which disappeared within 24 hours after administration of nonsteroidal anti-inflammatory drugs. Conclusions. Stimulating the healing of extensive granulating wounds in the perineal area with autologous lipoaspirate injection is a promising treatment method, promoting accelerated wound closure.
