Abstract.
Objectives. To determine the optimal volume of infusion-transfusion therapy (ITT) in patients undergoing anesthesia for ascending aortic and aortic arch surgery. Material and methods. We analyzed 43 cases of providing medical care to patients with acute ascending aortic and aortic arch dissection (type A). We divided patients into three groups based on intraoperative blood loss: the first group included patients with intraoperative blood loss of up to 30% of the circulating blood volume (CBV), the second group – with intraoperative blood loss of 31-70%, and the third group – with blood loss more than 70%. Results: In-hospital mortality in group 1 (n=10) was 40%, in group 2 (n=26) – 30.8%, in group 3 (n=7) – 71.4%. The volume 48 of ITT in group 1 was 2068 (1260; 2720) ml, in group 2 – 3665 (2960; 4030) ml, in group 3 – 5420 (4160; 6120) ml (p<0.05). The ratio of transfusion therapy (TT) components (erythrocyte blood components: fresh frozen plasma: cryoprecipitate: platelet blood component) in group 1 was 1:1:4:2.5, in group 2 – 1:1:2.5:1.5, in group 3 – the ratio was 1:0.75:1.5:0.8. Conclusions. The analysis of the quantity and quality of ITT as a component of anesthetic management during surgeries on the ascending aorta and aortic arch was conducted, which allows improving the treatment outcomes for this pathology.

Keywords: anesthesia, blood loss, ascending aortic dissection, infusion therapy, transfusion therapy.
