Experience in the treatment of lymphovenous insufficiency complicated by a trophic ulcer in the presence of severe concomitant pathology

Abstract.
Objectives. To evaluate the effectiveness of various approaches to the treatment of complicated lymphovenous insufficiency and the rate of healing of trophic ulcers in patients with various systemic diseases in the compensation stage.
Material and methods. The medical records of patients treated at the department of lymphology and chronic venous insufficiency (CVI) of the Center of Phlebology in Kaliningrad, Russian Federation, during the period from 2018 to 2021 were analyzed.
Statistical package SPSS 20.0 for Windows was used to automate the statistical processing of data.
Results. Two groups of patients were distinguished: the main group and the comparison group. After the given treatment, in patients of the comparison group, 73.3% (22 persons) had complete healing of the trophic ulcer, 13.3% (4 subjects) could not continue treatment, for reasons not related to their underlying disease, 6.67% (2 patients) received surgical treatment, 6.67% (2 patients) were sent to the hospital for skin flap transplantation. In patients of the main group – in 87.5% (21 persons) complete healing of the trophic ulcer was observed, in 4.12% (1 patient) with obliterating disease of the lower extremities and in 4.12% (1 patient) with obliterating disease and diabetic angiopathy the stage of decompensation developed, and the ulcer began to increase in size, 1 patient with diabetes mellitus in the compensation stage (4.12%) was referred for skin flap transplantation.
Conclusions. The healing time of trophic ulcers in patients with systemic concomitant pathology increases by 1.5±2.3 months on an average, compared with the control group of patients without severe concomitant diseases. However, with sufficient compensation of concomitant pathology, the trophic ulcer heals.
